Las secretarías de Salud del Meta y de Villavicencio, hacen un llamado a la población con discapacidad, para que gestionen ante las dos dependencias el acceso al certificado de discapacidad, con el cual podrán obtener importantes beneficios.
“Es un documento oficial, personal e intransferible, que acredita la condición de persona con discapacidad y facilita el acceso a beneficios como programas sociales, educativos, laborales y de salud, entre otros”, señaló Jorge Ovidio Cruz Álvarez, secretario de salud del Meta.
Este trámite, lo podrán realizar en las instalaciones de la secretaría departamental de Salud, ubicada en la vía que conduce al Hospital de Villavicencio, en la calle 37 #41-80 del barrio Barzal, en donde la secretaría de Salud del municipio recibirá la documentación a la población en condición de discapacidad, que les permitirá obtener el certificado.
Los documentos requeridos son:
Historia clínica con diagnóstico de discapacidad. Copia del documento de identidad.
Datos de contacto: Nombres y apellidos, número telefónico y correo electrónico.
Una vez radicados los documentos, se les entregará una autorización para la valoración física, a través de una IPS aliada, posteriormente deberán comunicarse y agendar cita de atención.
Por último, si la persona cumple con los requisitos médicos, recibirá el certificado de discapacidad y podrá empezar a disfrutar de grandes beneficios sociales.
